Book Chapter: Structural inequalities and chronic disease health outcomes: Implementing evidence-based strategies

This chapter on "Implementing Evidence-Based Strategies" examines why efforts to reduce health inequalities, despite focusing on individual and interpersonal approaches, haven't achieved lasting success. We argue that this persistent challenge stems from complex societal and economic factors that extend beyond an individual's control. We highlight the critical need for interventions that target broader systemic changes, as individual-level efforts, though important, often fall short on their own. We also explore practical strategies for putting evidence-based structural interventions into practice to address health inequities in adults living with chronic conditions.
